PROCEDURES FOR DRUG SCREEN COLLECTION AND BREATH ALCOHOL TESTING

1. Drug screen requests during Clinic hours. (8:00 am - 4:00 pm Monday Friday)
   1. The company places a call to the WorkCare clinic to request the test, stating the type to be done.
   2. If the employee has never been seen at WorkCare, informing the clinic personnel of the employee's information (i.e., name address, age, phone #, SS#, etc.) will help to expedite the registration process and save time.
2. Drug screen requests after clinic hours. (4:00 pm - 8:00 am Monday Friday weekends and holidays)
   1. Contact the switchboard at Trinity Medical Center West at (740) 264-8000 and they will contact us.
